Location This position is located in La Mesa, San Diego CountyHours 8 hour shift:

30 pm, working every third weekend Required Skills and Qualifications Current unencumbered California Registered Nurse licensure Current BLS certification for Healthcare Providers Three years RN experience One year experience as a Case Manager Basic knowledge of discharge planning Knowledge of clinical standards of discharge planning Knowledge of and experience with online computer systems Excellent communication skills Ability to communicate with patients, families, Physicians, and other disciplinary team members effectively Preferred Skills and Qualifications Bachelors of Science Degree in Nursing Certification in Case Management (CCM) Two years recent clinical experience in the area of specialtySummary This Per Diem Case Manager position is with the Case Management/Utilization Review/Social Service department at Sharp Grossmont Hospital.
The Case manager will partner with Attending and Hospitalist Physicians to ensure appropriate utilization resources determine patient type, and movement of patients to the next level of care.
Provide patients and family's education and information regarding the continuum of care process, discharge planning activities, allocation of resources, and long term planning.
Facilitates successful discharge through effective communication with patients, families, interdisciplinary team, payor source, post discharge service providers and referred community resources.
Sharp Grossmont Hospital utilizes a Case Management Model that is Physician-centric, patient focused service that coordinates, facilitates and advocates for the effective and cost effective delivery of high quality patient care.
The department is comprised of 40 plus employees to include RN Case Managers, Social Workers, and Coordinators.
The team provides a variety of support services to each patient and family for successful discharges.
Sharp Grossmont Hospital is the largest not-for-profit, full-service acute care hospital in the region with 542 beds.
